London: Yusuf Nazir’s Death: NHS Failed to Heed Mother’s Instinct, Report Reveals

لندن (پوٹھوار ڈاٹ کام، محمد نصیر راجہ – 04 اگست 2025): پانچ سالہ یوسف نظیر کی موت کے بعد جاری کی گئی نئی رپورٹ میں انکشاف کیا گیا ہے کہ برطانیہ کی نیشنل ہیلتھ سروس (NHS) نے ایک ماں کی فطری تشویش کو مسلسل نظر انداز کیا، جو یوسف کی جان بچانے میں معاون ہو سکتی تھی۔

یوسف نظیر، جسے روتھرہیم جنرل ہسپتال کے ایمرجنسی وارڈ سے علاج کے بغیر گھر بھیج دیا گیا تھا، آٹھ دن بعد پھیپھڑوں کے انفیکشن کے باعث انتقال کر گیا۔ 139 صفحات پر مشتمل NHS انگلینڈ کی رپورٹ میں کہا گیا ہے کہ یوسف کے خاندان کی بار بار کی گئی شکایات اور تشویشات کو صحت کے عملے نے تسلیم نہیں کیا۔

رپورٹ میں صحت کی دیکھ بھال کے عمل میں 13 مواقع کی نشاندہی کی گئی جہاں یوسف کی طبیعت بگڑنے کے باوجود کوئی مناسب قدم نہیں اٹھایا گیا۔

یوسف کی والدہ، سونیا احمد، نے جذباتی انداز میں بتایا:
“ہر رات جب میں آنکھیں بند کرتی ہوں تو یوسف کی بیبس آواز میرے کانوں میں گونجتی ہے کہ ‘ممی، میں سانس نہیں لے پا رہا، مجھے سانس نہیں آ رہی’۔”

انہوں نے کہا کہ ان کا بیٹا “میرے سامنے جان سے گیا، درد سے کراہتا رہا مگر اسے درد کی دوا تک نہ دی گئی۔”

سونیا احمد نے مزید کہا:
“ماں کی فطری حس کو نظر انداز کیا گیا، حالانکہ یہ ایک طاقتور جذبہ ہوتا ہے۔ یوسف بہتر علاج کا حقدار تھا، ہر بچہ ہوتا ہے۔ میں اس کا نام لینا کبھی نہیں چھوڑوں گی، نہ ہی انصاف کی جنگ۔ ہم جوابدہی چاہتے ہیں، ہم تبدیلی چاہتے ہیں۔”

NHS نے رپورٹ میں پیش کردہ سفارشات کو تسلیم کرتے ہوئے ان خامیوں کو تسلیم کیا ہے جو اس تفتیش کے دوران سامنے آئیں۔

London (Pothwar.com, Mohammad Naseer Raja – 04 August, 2025): A new report into the death of a five-year-old boy, Yusuf Nazir, who died days after being sent home from a hospital emergency department,t has found the NHS failed to listen to a “mother’s instinct”.

Yusuf Nazir died eight days after he was prescribed antibiotics by staff at Rotherham General Hospital to treat a lung infection.

The 139-page NHS England report said the concerns of Yusuf’s family were repeatedly not acknowledged by healthcare providers.

The health service said it accepted the report’s recommendations and acknowledged the issues the investigation had uncovered.

Yusuf’s family has called for a full inquest and insisted that the NHS learn lessons from his death.

His mother, Soniya Ahmed, said: “Every night when I close my eyes I hear Yusuf’s helpless voice in my ears saying, ‘Mummy, I can’t breathe, I can’t breathe, I really can’t’.

“And the image of him being handed over to me after his life ended, as though someone has torn my heart out and placed it in my hands.”

She said her son was “left to die right beside me”.

“He was crying in pain, yet he received no pain relief.

“They say a mother’s instinct is a powerful thing; it can neither be ignored nor explained, yet my concerns were ignored each and every time.”

Ms Ahmed described the failings in his care as “truly shocking”, citing 13 opportunities the report outlined to escalate her son’s care.

“Yusuf deserved better; every child does. I will never stop speaking his name. I will never stop fighting for him. We demand accountability, we demand change,” she added.
